La violencia de pareja hacia las mujeres es un problema de salud de primer orden con graves repercusiones para la salud física y mental de las víctimas y de los conviventes, y de esta forma ha sido recogido por las principales organizaciones con competencias en salud.

This study explored perceived benefits and risks of participation in HIV research among 33 female sex workers in Tecún Umán, Guatemala. Stigma associated with sex work and HIV was a critical barrier to research participation. Key benefits of participation included access to HIV/sti prevention and testing, as well as positive and trusting relationships between sex workers and research teams. Control exerted by managers had mixed influences on perceived research risks and benefits. Results underscore the critical need for HIV investigators to (...) develop population-tailored procedures to reduce stigma; engage managers; and reinforce trusting, reciprocal relationships between sex work communities and researchers. Abstractabstract:This article analyzes Claude McKay's last novel, Amiable with Big Teeth—recently discovered in 2017—as a piece of postcolonial utopianist writing. The novel participates in an important debate on the role of utopias and utopian writing as ideological mechanisms that perpetuate colonial structures such as the nation-state. Through a critique of the Popular Front project in the black community of 1930s Harlem, Amiable with Big Teeth vindicates local knowledges and the assessment of the specific conditions of the present—ever-changing and transformable—in the (...) development of strategies for resistance. As such, the spiritual role of Ethiopia for the community depicted, rather than constitute yet another national utopia, fulfills the same role as it did for the Jamaican Rastafarians: it is a mechanism for group self-assertion and the promotion of self-esteem that subaltern communities need in order to achieve full agency. Background: Serious games have shown positive results in increasing motivation, adherence to treatment and strengthening the therapeutic alliance in multiple psychiatric disorders. In particular, patients with impulse control disorders and other disorders in which the patient suffers from inhibitory control deficits have been shown to benefit from serious games.Aim: The aim of this study was to describe the characteristics and to evaluate the usability of a new serious videogame, e-Estesia. This serious videogame was designed to improve emotion regulation in patients (...) with gambling disorder. Preliminary results from a pilot sample are also reported.Method: A pilot sample of 26 patients undergoing treatment for GD was recruited. Participants used e-Estesia on a tablet, which was connected to a thoracic band that sent heart rate and heart rate variability data to the videogame platform in order to provide biofeedback. The System Usability Scale was completed by patients to determine the usability of e-Estesia.Results and Discussion: e-Estesia performed comparatively well for all the explored groups. Around 84.6% of the patients endorsed that it was easy to use. Female patients with GD presented higher HRV during the use of the serious videogame compared to men. (shrink)

IntroductionRheumatologists are the primary healthcare professionals responsible for patients with rheumatic diseases and should acquire medical ethical competencies, such as the informed consent process (ICP). The objective clinical structured examination is a valuable tool for assessing clinical competencies. We report the performance of 90 rheumatologist trainees participating in a station designed to evaluate the ICP during the 2018 and 2019 national accreditations.MethodsThe station was validated and represented a medical encounter in which the rheumatologist informed a patient with systemic lupus erythematosus (...) with clinically active nephritis about renal biopsy. A trained patient–actor and an evaluator were instructed to assess ICP skills (with a focus on kidney biopsy benefits, how the biopsy is done and potential complications) in obtaining formal informed consent, delivering bad news and overall communication with patients. The evaluator used a tailored checklist and form.ResultsCandidate performance varied with ICP content and was superior for potential benefit information (achieved by 98.9% of the candidates) but significantly reduced for potential complications (37.8%) and biopsy description (42.2%). Only 17.8% of the candidates mentioned the legal perspective of ICP. Death (as a potential complication) was omitted by the majority of the candidates (93.3%); after the patient–actor challenged candidates, only 57.1% of them gave a clear and positive answer. Evaluators frequently rated candidate communications skills as superior (≥80%), but ≥1 negative aspect was identified in 69% of the candidates.ConclusionsEthical competencies are mandatory for professional rheumatologists. It seems necessary to include an ethics competency framework in the curriculum throughout the rheumatology residency. (shrink)

SummaryA number of authors have identified multiple concurrent sexual partnerships by both men and women to lie at the root of the HIV/AIDS epidemic in sub-Saharan Africa. This study applies multilevel models to Demographic and Health Survey data collected during 2003–2008 in 20 sub-Saharan African countries to examine the influence of social and cultural context on involvement with multiple sexual partnerships in the region, above and beyond the effects of individual characteristics. The findings provide support for the ecological argument that (...) health behaviours are shaped and determined by societal conditions, in addition to the effects of individual and household characteristics. Involvement with multiple sex partners is most prevalent in societies in which sexual norms are widely permissive and where polygyny is common. Individual autonomy is substantial and attitudes towards sexuality are more liberal among men and women who live in communities in which sexual norms are widely permissive. Men and women who are most likely to have multiple sex partners in the sub-Saharan region are those who initiated sexual activity earlier and those who have the individual attributes that bring to them more rights and/or decision-making autonomy, but not necessarily more financial resources and economic security. On the other hand, involvement with multiple partners is determined by cultural norms and social change. The findings suggest a number of opportunities for more effective policy and programmatic responses to curb the prevalence of multiple partnerships in sub-Saharan Africa. (shrink)

A serious problem confronting discourses on recognition is that of showing equal respect for citizens’ diverse cultural identities whilst at the same time attending to feminist concerns. This article focuses on the complex issues emerging from the recent legislation prohibiting the Muslim veil in French state schools. I respond to these problems by defending two conditions of a postcolonial and feminist approach to the politics of recognition. This approach should be, first, transformative, in the sense of widening its conception of (...) core values through an engagement with cultural difference. Second, it should be critical in its orientation to practices affecting women adversely within any social group. An integration of these concerns is proposed in terms of ‘affective citizenship’. This approach supports the different components of women’s autonomous functioning, through a universalistic commitment to the creative expression of their hybrid identities. (shrink)

In this issue ofJME, Watsonet alcall for research evaluation of government health programmes and identify ethical guidance, including the Ottawa Statement on the ethical design and conduct of cluster randomised trials, as a hindrance. While cluster randomised trials of health programmes as a whole should be evaluated by research ethics committees (RECs), Watsonet alargue that the health programme per se is not within the researcher’s control or responsibility and, thus, is out of scope for ethics review. We argue that this (...) view is wrong. The scope of research ethics review is not defined by researcher control or responsibility, but rather by the protection of research participants. And the randomised evaluation of health programmes impacts the liberty and welfare interests of participants insofar as they may be exposed to a harmful programme or denied access to a beneficial one. Further, Watsonet al’s claim that ‘study programmes … would occur whether or not there were any … research activities’ is incorrect in the case of cluster randomised designs. In a cluster randomised trial, the government does not implement a programme as usual. Rather, researchers collaborate with the government to randomise clusters to intervention or control conditions in order to rigorously evaluate the programme. As a result, equipoise issues are triggered that must be addressed by the REC. (shrink)

Background The early phase of the COVID-19 pandemic affected nursing homes and their residents heavily. Guidelines on how to mitigate the virus’s spread and ensuring safe healthcare delivery were continually evolving. Concurrently, nursing homes faced intense media scrutiny. This challenging environment severely impacted registered nurses and physicians employed within these facilities. Aim To understand the ethical challenges experienced by registered nurses and physicians working in nursing homes during the COVID-19 pandemic. Research design Qualitative descriptive research using thematic analysis. Participants and (...) research context Individual online interviews with four registered nurses and eight physicians clinically active at nursing homes in Sweden. Ethical considerations The study was approved by the Swedish Ethical Review Authority. All participants provided written consent. Results Registered nurses and physicians working in nursing homes perceived ethical challenges stemming from early COVID-19 pandemic guidelines and media coverage. The main theme generated was ‘Struggling to maintain professional and ethical standards under pressure’ incorporating two subthemes: ‘Guidelines developed without the profession put pressure on staff’ and ‘Media’s biased reporting was perceived as unethical and undermined care’. Guidelines from the authorities were considered as developed without professional involvement. It made them difficult to adhere to without deviating from professional and ethical compasses. Media coverage adversely influenced relatives’ perceptions, resulting in mistrust towards physicians’ and registered nurses’ in delivering optimal care for the residents. Conclusions Resilient care in nursing homes necessitates the collaborative development of guidelines involving registered nurses and physicians, particularly amidst crises. Moreover, it is vital to provide support to registered nurses navigating ethical dilemmas, especially during pandemics. Guidelines and principles for care during a crisis should be development with professional involvement, be transparent, and be available to the public, to promote neutral media coverage. Future research is crucial to enhance ethical standards and tackle challenges in this context. (shrink)

The European Union faced high risks from personal data proliferation to individuals’ privacy. Legislation has emerged that seeks to articulate all interests at stake, balancing the need for data flow from EU countries with protecting personal data: the General Data Protection Regulation. One of the mechanisms established by this new law to strengthen the individual’s control over their data is the so-called “right to be forgotten”, the right to obtain from the controller the erasure of records. In gender transition, this (...) right represents a powerful form of control over personal data, especially health data that may reveal a gender with which they do not identify and reject. Therefore, it is pertinent to discern whether the right to have personal data deleted—in particular, health data—is ethically acceptable in gender transition. Towards addressing the ethical dimensions of the right to be forgotten in this case, this study presents relevant concepts, briefly outlines history, ethics and law of records considering the evolution from paper to electronic format, the main aspects of identity construction and gender identity, and explores the relationship between privacy, data protection/information control and identity projection. Also, it discusses in gender transition the relation between “the right to self-determination”, “the right to delete”, and “the right to identity and individuality”. Conclusions on the ethical admissibility of the ‘right to be forgotten’ to control gender-affirming information are presented. (shrink)

This chapter compares the institutional setting and integrations processes in Switzerland and the EU. The major findings are that EU integration is trying to achieve more political integration and accommodation of a much higher degree of diversity in much less time than has ever been the case in Switzerland. Integration and expansion processes that were slower and non-linear in Switzerland and that happened in separate phases (e.g. religious diversification, linguistic diversification, territorial expansion, etc.) are all going on at the same (...) time in the EU. Especially integration and accession with enormous shocks of diversification are engineered at the same time in the EU. From this point of view, the EU has already tried to go beyond many stages that took centuries to be completed in Switzerland. The institutional design of the European Union seems to echo quite well the federal state formation process in Switzerland. The following precisions are however necessary in the comparative perspective. First, the momentary stage of European Integration, characterized by intergovernmental crisis management, resembles the (dysfunctional) intergovernmental centralism of the Swiss cantons during the decades before the formation of the federal-state in 1848. Second, due to the greater diversity of the European Union, this quasi-federal system has derived in extreme asymmetries between the member states. Since EU identity is not well entrenched among European citizens (and politicians), it has been hard to design institutions and policies of common territorial protection and redistribution and there is mistrust towards centralistic EU institutions (specially in the countries more affected by the economic crisis). Most European citizens do not feel that their interests are taken into account by the European Union. Third, it is important to note that in Swiss federalism the municipalities play an important role, they are much more than just administrative districts. This city-centred and bottom up construct of citizenship is guaranteed by the Swiss federal constitution. Citizens feel that their most immediate and local identity is not jeopardized but rooted in and guaranteed by the Swiss federal constitution. Compared to sub-national Swiss federalism, EU federalism is entirely focused on the nation-state and the EU institutions. Serious consideration ought to be given to the idea that European citizenship is not only about bringing citizenship to a higher European level but also about bringing it more to the root-level of citizenship: the city. Direct democracy has acted as a federator in the Swiss context. Switzerland made direct democracy and direct democracy made Switzerland. There has been a slow and iterative process of adaptation of structurally similar institutions of direct democracy at all levels (communal, cantonal, federal) of all units (all communes, all cantons, confederation roughly between 1830-1891. To the contrary, the EU is only incipiently in a process of introducing direct democracy (in some member-states and ECI), and so far direct democracy is mainly practiced as national plebiscitary democracy. Under this guise it is seen as a threat to EU integration and probably not without good reason. While in Switzerland the coherent introduction of direct democracy at all levels of the polity in the long run served as an important unifier, direct democracy has even not been considered as integrative part of all levels of political integration in the EU. It is of great interest that the one element in which the European Union has based the construction of EU citizenship and identity – mobility of residence – has been implicitly discouraged in Switzerland. The institutional design as incorporated in Swiss multicultural identity (which aims fundamentally at the protection of cantonal autonomy, culture and language) has facilitated that Switzerland is called today a successful multicultural society. Most citizens identify with Switzerland as a country and they like it as it is, but they do not want to take advantage of their formal right to move to other parts of the country, especially not across language borders. The same institutional design that has made of Switzerland a successful case of multiculturalism and democracy poses important barriers that make it difficult for the Swiss to move their residence across their country. Considering that one of the main features of European citizenship is the freedom of movement and residence, this poses a main concern. The Swiss compromise between the formal right and economic necessity of mobility on the one hand and the protection of political and cultural sub-identities on the other hand, is commuting. Due to the vast size, this is of limited applicability in the EU. However, in a Europe of cities and trans-border regions, commuting is an important option provided that every European citizen lives reasonably close to an important economic centre. In short, the Europe of commuters deserves attention in the context of EU citizenship. (shrink)

Andrés de Guevara y Basoazábal (1748-1801), jesuita novohispano guanajuatense, escribió las Institutionum elementarium philosophiae, donde su autor ofrece un currículo deseable para la juventud mexicana que incluya la ciencia moderna empírica, la política virtuosa y la sabiduría humana. A través de una traducción comparativa del latín al español del proemio de la citada obra, este artículo pretende: 1) contextualizar al autor novohispano, 2) mostrar la recepción de la ciencia moderna desde los elementos aristotélicos de los que Guevara dispuso, y 3) (...) mostrar que la modernidad y la tradición aristotélica se enriquecieron mutuamente; y lo que debía de reformarse era la escolástica anquilosada. Se concluye que a través del proemio De vicissitundinibus de Guevara se evidencia la recepción mexicana de la modernidad emanada de una mentalidad aristotélica. (shrink)

Y tiene como objetivo el tratar de dilucidar dentro de la misma inmanencia de la experiencia trascendental del yo pienso, el modo no solo como surge la intersubjetividad, sino cómo este último concepto al igual que el de la egología, tienen como fundamento una noción de subjetividad moderna que por un lado, aparece dentro de la arquitectónica fenomenológica, como constituyente de sentido dentro de la (...) meditación husserliana, y por otro, puede aparecer como una noción que impide la elaboración de un nuevo concepto de hombre que incluya en su seno la alteridad, tal y como piensa Emmanuel Lévinas, y con ello, la posibilidad de hablar de un yo separado que no encuentre en lo otro una mera analogía suya, o extensión de su propia inmanencia, sino antes bien, la posibilidad de hallar en eso otro que nos sale al paso, la condición por la que se puede hablar de tiempo, de mundo y de existencia como relación con otros y para otros. The advantage of examining causality from the perspective of modelling is thus that it puts us naturally closer to the practice of the sciences. This means being able to set up an interdisciplinary dialogue that contrasts and compares modelling practices in different fields, say economics and biology, medicine and statistics, climate change and physics. It also means that it helps philosophers looking for questions that go beyond the narrow ‘what-is-causality’ or ‘what-are-relata’ and thus puts causality right at the centre of (...) a complex crossroad: epistemology/methodology, metaphysics, politics/ethics. This special issue collects nine papers that touch upon various scientific fields, from system biology to medicine to quantum mechanics to economics, and different questions, from explanation and prediction to the role of both true and false assumptions in modelling. The “context of discovery” and “context of justification” distinction has been used by Noretta Koertge and Lynn Hankinson Nelson in debates over the legitimacy of feminist approaches to philosophy of science. Koertge uses the context distinction to focus the conversation by barring certain approaches. I contend this focus masks points of true disagreement about the nature of justification. Nonetheless, Koertge raises important questions that have been too quickly set aside by some. I conclude that the context distinction should not be (...) used to block feminist philosophy of science because the use of the context distinction is deeply ambiguous, masking underlying debates about naturalism and the nature of justification. ArgumentUsing the notion of styles of knowledge we refer to the ways diverse scientific communities claim to produce true knowledge, their understandings regarding the attitudes and values that scientists should have in order to grasp natural and social reality, and the practices and technologies developed within such styles. This paper analyzes scientific and medical enterprises that explored the relationship between environment, population, and society in Colombia between 1850 and 1920. We argue that similar styles of knowledge production were shared in (...) human geography, medical geography, and climatic physiology at the mid-nineteenth century; and that some physicians working in bacteriology and physiology since the 1880s established epistemic boundaries between their work and earlier scientific activities, while others found these distinctions irrelevant. However, the historical actors committed to any of the styles of knowledge production explored in this article agreed on the local specificity of their objects of inquiry, therefore questioning European science. These styles of knowledge production also shaped different ways of perceiving and addressing national problems. Hence, this article is a contribution to the recent literature on both historical epistemology and social and cultural history of science and medicine. (shrink)

A lexical analysis of the so-called Kanuni i Skanderbegut, an Albanian collection of customary laws, provides an insight to understand women’s social role in traditional communities living in the northern area of Albania. In the use of specific terminology one can notice a clear separation between the two sexes which is reflected in the distribution of roles and in using a structured terminology that designates not only roles and conditions, but also other aspects of social life.

This paper is concerned with the bias in favor of the interests of the members of some species of non- human animals and against the interests of the members of other species of non-human animals. This view, which I call modified speciesism, is perhaps related to Singer’s speciesism, but neither entails nor is entailed by it. The argument takes the following form: given that exploited animals are morally equivalent to non-exploited animals and given that non-exploited animals are morally entitled to (...) the way that we treat them, exploited animals are entitled to such treatment as well. I will give a descriptive account of how modified speciesism is prevalent in our world today which serves as at least a partial defense of the first premise. I will then give a Humean defense of the second premise based on the notion that it is wrong to inflict unnecessary pain and suffering on animals. (shrink)

Recent events, by making necessary online classes and exams, are raising plagiarism and cheating practices among students. I offer strategies to deal with both issues at the stake. I argue that institutions and educators should not only focus on how to detect bad practices and punishments, but rather on preventing despicable behaviours through the construction of a more sophisticated educational system.

In The Gay Science, Book I, Aphorism 7, which is entitled “Something for the industrious”, Nietzsche points to a “work program” to the new gay science, programm that move away philosophy from usual places with which it is familiar. Likewise, in The Gay Science there are mentions about a “natural history of the animal” and, given the link between the birth of moral and our relation with animals (as observed in Human, all too Human II, 57), I consider that part (...) of “this something” for the industrious in present times must be carried out regarding the animal issue. Then I will outline some propositions from Nietzschean philosophy in order to think this subject, mainly on the basis of the criticism and complaint of those humanists prejudices that continue to form a “naturalized” part of our link with animals.
